Please read carefully: Before any event can be scheduled at FBC, this form must be filled out and approved by the church staff. Be sure to look over the entire form and to fill out any areas that will pertain to your event. Your event is NOT scheduled until the church secretary notifies you that the church staff has approved your date, purpose, and arrangements.
